Expectations & Agreements Checkbox I understand this service agreement must be completed prior to commencement of mentoring. I understand that mentoring sessions require 24 hours’ notice to cancel, or the session will be invoiced in full. I will invite the mentor to relevant care team meetings. I will assist with adding the mentor to the call list if the young person is in custody. I will support urgent or short-notice contact if required while the young person is in custody. I will ensure the young person understands that the mentor is not a worker — they are a mentor, independent of care team duties. I will share any mentoring-related plans, strategies, or documents with the mentor directly before decisions are made. I acknowledge that mentors may at times have pre-existing community or cultural ties to the young person or their network, and that these relationships are often valuable and strategic. I understand that all mentors are held to a high standard of professionalism. 7. Additional Notes or Questions By submitting this form, you confirm your agreement to the service terms outlined above.
