Fill in this paysheet each week. Include the mentoring sessions you completed, any double sessions, and any cancellations you can claim payment for.

Submit your paysheet and all completed session reports by 4 pm on Monday for the previous week. If you submit them late, your payment will move to the following week.

Cancelled sessions: You can only claim payment if the session was cancelled in the 24 hours before it was due to start.
